I think the Form is good!. Help us to provide a better service to the claimants. I would like to offer some of my ideas in how to improve a little bit the form: In the section where says "Program" add another option for PUA and UI some claimants has both. In the "personal Information" separate the information of the caller and person that appears on the 1099. For example, some people call to report that they receive a 1099 in the name of a person that does not live in that address.

Found Cognito forms to be the easiest free form creator that I have used. Was able to use conditional logic for sub-forms within my forms. Furthermore, the results of the online data gathering are easy to read and can be downloaded easily into a spreadsheet or database.

It is a better solution than many others I have tried out.
The primary shortcomings are:
- the lacklustre UI/UX for editing the forms;
- the over-complicated UI for the notification/confirmation emails;
- the lack of easily templatable/repeatable features or sections between forms;
- the complexity in creating pre-filled forms based on a previous form's input (the need to "manually create" the link makes this unwieldy for anything but the most simple pre-filled forms) and changing one the first form requires then changing the second one -- it would be great to be able to "connect" questions/answers from 2 distinct forms together.
Some of these shortcomings seem to be shared with your competitors.

I love the product. The only 2 issues have is the ability to white label and the ability to change the link to the form. The link is automatically created from the name of the form and it can be problematic if you decide to change the title or if the title is to long.
