I've tried various form systems over the years and settled with Cognito 2 or 3 years ago. My hundreds of forms range from the very simplest to complex stock-driven forms collecting money through Stripe. They are intuitive, powerful and backed up by excellent support.
I always recommend them through forums and online chats and I can't see myself changing anytime soon.

Hassle-free, simple-intuitive UI UX. The template designs is sufficient IMHO, not to much but quite vary.
a little problem in the first time is login problem where after logged in, you kept being kicked out to home, which I already submit to the troubleshooting team and turned out that the cache must be deleted first. And now that problem never occur again, ever.
There's room for improvement though, where it can be added with positive/negative auto reply response based on the chosen answer (in multiple answer version).
But other than that, I am satisfied. Well done, Cognito!
